DISTINCTIVE FEATURES
Occurs in postmenopausal women.
Patient may have history of radiation.
Manifests with endometrial bleeding.
Mesenchymal tumors of the endometrium may be composed of primitive stromal cells which are derived from the mullerian mesoderm.
The mesenchymal tumors of the endometrium are three types: benign stromal nodule, low grade stromal sarcoma (endolymphatic stromal myosis) and endometrial stromal sarcoma.
Benign stromal nodules consist of stromal cells within myometrium. The low grade stromal sarcoma also consists of stromal cells with the capability to penetrate the lymphatics. The stromal sarcoma consists of overtly malignant stromal cells. These cells exhibit atypia, and numerous mitoses. The sarcomatous stroma may surround carcinomatous glands. This type of tumor is called carcinosarcoma or homologous mixed mesodermal tumor. In some tumors, the stromal cells may differentiate into other types of mesenchymal cells such as smooth or striated muscle, adipose tissue, cartilage or osteoid. This type of the tumor is called heterologous mixed tumor.
